IMPORTANT – Please note:

  • This form must be completed for all leave requests.
  • For leave requests of more than one week a minimum of two (2) weeks notice must be given to management.
  • For leave requests of less than one week a minimum of three days must be given to management
  • Please be aware that leave requests are to be negotiated with management and while we will endeavour to ensure your request is successful there may be times when we cannot accommodate your request.
  • Do not book travel and/or accommodation until you have spoken with management.
